In your role as a Practice Transformation Coach with the DVACO (Delaware Valley Accountable Care Organization), you will study and monitor care processes in a quality infrastructure and use data to drive ongoing performance improvement throughout the DVACO. You will be responsible for coordinating, interpreting, and translating performance data; and critically reviewing metrics.

- Get Involved! Collaborate with DVACO Leadership team on ongoing opportunities for improvement in Clinical Operations, Quality, Risk Reduction, Patient Outcomes; this may be in response to regulatory, payer, entity & health system requests as it relates to clinical quality.

Position: Practice Transformation Coach - DVACO
Shift: 40hrs/wk
Experience:
1. Working knowledge of ambulatory care service delivery, clinical operations/ workflows, and clinical data analysis and reporting desired.
2. Strong/solid knowledge and skill with computers, spreadsheets and databases required.
3. Proficiency in software applications that include, but are not limited to Microsoft Word, Excel, and PowerPoint.
4. Working knowledge of EMR (Electronic Medical Records) desired.
Education:
1. Bachelor's degree in related field with prior applicable experience strongly preferred.
2. In lieu of degree, at least 5 to 7 years of relevant experience is required.
Licensures/Certifications:
Patient-Centered Medical Home Certified Content Expert (PCMH CCE) preferred but not required.
