Per Diem,Case Manager Home Care

As a Per Diem Case Manager in Home Care, you will be responsible for the coordination of services, progress, and personnel necessary to facilitate a certain group of patients through the continuum of care utilizing appropriate resources. You will also collaborate with the multidisciplinary team to provide a coordinated experience to patients and families, and participate in quality improvement and evaluation processes.

Position: Case Manager - Home Care
Shift: Per Diem. One Weekend Per Month Required. Additional Hours Available Monday - Friday.

Experience:
1. Minimum of three (3) years inpatient experience in medical Case required.
2. Management, Utilization Review, Discharge Planning or other appropriate
health-related field required.

Education:
1. Graduate of an accredited School of Nursing.
2. Bachelors degree or in
progress required.
3. MSN preferred.

Licensures/Certifications:

1. Current licensure in the Commonwealth of PA as a Registered Nurse required.

2. Case Management certification preferred.
