Make an Impact! The Wound Ostomy Continence Nurse (WOCN) serves as a clinical resource and consultant for home health and hospice patients with selected disorders of the gastrointestinal, genitourinary, and integumentary systems including (but not limited to) acute or chronic wounds, ostomies, and continence disorders. Utilizing the nursing process, the WOCN provides hands-on care and educates clinicians, patients, and their families to address physical, psychosocial, and spiritual needs of patients and families to improve function, independence, and quality of life. The WOCN demonstrates critical thinking, analysis, and clinical judgment in all aspects of care delivery for patients and caregivers facing acute, chronic, and life limiting illness. The WOCN collaborates and coordinates care with nurses, the physician, and other health care team members to implement the plan of care and meet the rehabilitation needs of patients with wound, ostomy, and continence needs. Additionally, the WOCN performs other related tasks for documentation and supply management. Professional responsibilities are carried out through direct patient care, clinical consultation, education, research, and administration.

Position: Wound Ostomy Continence
Nurse (WOCN) – HomeCare & Hospice
Shift: Monday - Friday, Weekends as Needed
Experience:
1. Two years relevant med-surg experience in an acute or long-term care
setting, rehabilitation, home care, or hospice setting preferred.
Education:
Graduate
of NLN approved school of nursing. Bachelor’s Degree in Nursing required.
Graduation from a WOCN-accredited WOC nursing education program.
Licensures/Certifications:
1. Current
license to practice nursing in Pennsylvania.
