- Make an Impact! As the Case Manager, Hospice this employee is responsible for facilitating the patient through the continuum of care by coordination of services, programs and personnel. Promotes the achievement of patient-centered goals with the use of appropriate resources. Collaborates closely with system multidisciplinary teams to provide a coordinated experience for patients and families. Participates in quality improvement and evaluation of processes. As the primary care nurse this employee provides direct patient care including support and education of both the patient and family in the hospital. The nurse coordinates the case management among the hospice and hospital teams, utilizing additional resources as appropriate, in accordance with the mission, objectives, policies and procedures of the health system.

Position: Weekend Inpatient Hospice Nurse Liaison
Shift: Day Shift; 2 out of 4 weekends per month OR 3 out of 4 weekends per month; Saturday & Sunday
Experience:
Three to five years nursing experience with at least two years hospice experience Basic computer skills required and familiarity with resources within the health system and community
Education:
RN Diploma, ASN required; BSN preferred.
Licensures/Certifications:
1. Current license to practice nursing in PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
2. Valid Driver's license and automobile insurance.
3. Provider BLS (American Heart Assoc/American Red Cross) preferred.
4. CHPN preferred.
Requirements:
This position requires you to wear a respirator as part of the job requirement.
