Home Health Aide,HCN
The home health aide is a paraprofessional member of the home care and hospice teams who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient�s needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed. Education: High school diploma is preferred or clinical rotation in nursing school Successful completion of a training program and competency evaluation that meets Medicare Conditions of Participation for Home Health and Hospice agencies. Successful completion of pre-employment home health aide skills assessment test with score of 76 percent. Ability to read and follow written instructions and document care given. License & Certifications: Valid driver�s license and automobile that is insured in accordance with organizational requirements and is in good working order. Certified Nursing Assistance (CNA) preferred. Certified Hospice and Palliative Nursing Assistant (CHPNA) preferred. Current CPR certification preferred.
