Main Line HomeCare and Hospice is currently seeking a Occupational Therapist to be a Care Team Coordinator (CTC) to be responsible for a client care team, which serves the JHH-MLH-Home Care Agency. This individual is responsible for coordination of clinical care across the disciplines and health care providers. The CTC works in conjunction with the Operations Manager to develop and monitor client care services based on customer needs. The CTC oversees the day to day operation of the care team. The CTC reports to the Operations Manager.
Education: Bachelors degree in allied health field required, masters degree preferred. 3-5 years recent clinical experience in an allied health profession or home care required. Management/Supervisory experience preferred
Licensures & Certifications: Licensed in Commonwealth of PA in Occupational Therapy with a current CPR certification. Valid Drivers license and insured automobile (as applicable).
Experience: Relevant clinical experience as per setting
